Department of Insurance at 322 Chapanoke Rd., Raleigh, NC 27603 Monday through Friday from 8 a.m. to 5 p.m. Payment options include Visa, MasterCard, personal check or money order. Go to the NCDOI Engineering and Codes Web site and print the order form PRIOR to going to the NCDOI offices.
